Policies
Rescheduling, Cancellation, & No-Shows
Dear Coastal Spa MD patients, the following policy document will be reviewed with you and must be signed by you at your initial appointment (new patients) or your next available appointment (previous patients):
- Please try to arrive 5-10 minutes prior to your schedule time, so you can receive your full appointment time ensuring the best possible outcome, and staff members have enough time to prepare for following appointments.
- Paying credit card information will be documented during initial appointment intake (for any future infractions) and will be kept on file confidentially.
- If you need to cancel your appointment, please contact us within a 24-hour notice, so we have the opportunity to offer your appointment to another patient.
- All appointments cancelled less than the 24 hour-notice/same day cancellation, you will be charged a $25 cancellation fee with the card kept on file.
- If you do not show up for a scheduled appointment, you will be charged a $50 cancellation fee.
- If you are late for an appointment, you will be seen as soon as possible, and for the length of time remaining to your appointment. Lateness 15 minutes or greater will be charged a $25 late fee and your appointment will be rescheduled.
- Coastal Spa MD and staff reserve the right to refuse patients who demonstrate repeat rescheduling, cancellations, and no-shows and will be charged accordingly.
- I understand the terms and conditions of this form. I realize I am financially responsible for any charges incurred from cancellations/no-shows/rescheduling and Coastal Spa MD and its staff will not be held liable.
Refunds
All services and treatments at Coastal Spa MD are non-refundable, but certain situations will vary patient to patient. Treatment results will vary patient to patient, and aside from certain touch-ups or follow-up appointments, any additional treatments needed to achieve desired results will be priced as an additional treatment.
